There is a problem that keeps happening randomly throughout the program

Throughout the program in many quizzes in many different topics I am getting red X´s for answers that were not my answers and I am not receiving credit for my correct answers. I have seen this happen many times (in 500 tests maybe 25 times) and I finally have concluded that it really is happening but unfortunately I cannot give the feedback that you require, namely, the location where it happened, because by the time I detect a problem, the original question is already gone from the screen and cannot be retrieved. I realize that it would be very difficult if not impossible to locate and solve this because of the sheer size and complexity of the program. I understand this as a former professional software developer but I still feel that I need to report it. 

This is not a complaint, per se, but merely an informative comment for your consideration.

Hi James - thanks for reporting this. Could you look back through your kwiz history (bottom of your dashboard) and find an example of a test report where this occurred? If you could send a snapshot into support explaining what you were expecting to see versus what the report showed then we'll be able to look into this for you.

I'm not sure I understood what you meant by "the original question is gone already" as the question is shown in the marked answers report so if you could clarify that as well when you write in that would be very helpful for us to understand what's going on.

I have now taken over 800 quizzes. I have not kept track of these instances and It is not reasonable for me to review those 800 quizzes looking for examples of this problem. Think about it, would you want to review 800 questions looking for 20 or so cases where the answer was not graded correctly? Therefore, I am unable to provide the information that you request. I have now decided to accept these anomalies and proceed onward. The primary consequence of this is that I will need to take a few additional quizzes here and there to compensate for these errors and reach the 100% goal for each level.

Hi James, no worries, I assumed that as you'd decided to write in that it had happened recently enough to be in the last few kwizzes. Certainly not worth trawling through any more than that but if you do notice it happening again and you remember to, then grab a screenshot and send it to support and we can take a look.
