I don't understand the use of menudo here because it means small and the question uses very often not very small. Can you explain this to me?

The adjective "menudo" means "small", you are right. But here we are using "a menudo", which is an adverb meaning "often". That is the meaning in this sentence:
Alberto llora muy a menudo.
(Alberto cries very often.)
