They seem to be interchangeable
Si usted hubiera venido antes, yo le habría atendido.If you had come before, I would have served you.
Ella hubiera venido.She would have come.
Ella habria venido.

Not sure exactly what you mean, I guess you mean that that "you" (usted) could be referring to a man or a lady? If that's the case, yes, it could be both. 

Or maybe you are referring to the form of the verbs that are the same for usted, for él, for ella, for yo...: hubiera / habría
