"Él la respeta a ella pero no respeta a sus hijos."
Why wouldn't it be "no los respeta a sus hijos"?

The repetition of the pronoun works when there is a "a + pronoun", e.g. a mí, a ti, a él...., but not with a common noun or a proper noun; so we could say:
"Él la respeta a ella pero no los respeta a ellos."
but we cannot say:
"Él la respeta a ella pero no los respeta a sus hijos."
"Él la respeta a ella pero no los respeta a Javier y Carlos."
