Que vs. que

Are there structural clues we can look for to determine whether "que" is meant to express a wish vs. disbelief or would it be contextual? I ask since sometimes the disbelief version can be followed by the subjunctive.

It is contextual but when we use "que" to express a disbelief/surprise, you normally find this in the form of a question:

¿Que te han despedido?

Are you telling me you've been fired?/ I can't believe you've been fired!
