In the tables, you use tuvierais and tuvieseis. In the examples you use tuviérais and tuviéseis. Is this a typo or is there another reason for changing the inflection?

The key difference is this:

Nosotros form → tuviéramos / tuviésemos (WITH an accent)
-
Vosotros form → tuvierais / tuvieseis (NO accent)
Only the nosotros form needs the accent mark because in tuviéramos and tuviésemos, the stress falls on -vié-, and Spanish spelling rules require the accent to show that stress.
In tuvierais / tuvieseis, the stress naturally falls in the correct place, so no written accent is needed.
